Co-Performer Release Agreement

What to Know

This is a form you can use to ensure you have obtained written consent and age verification from a co-performer who is not already a model on SextPanther.

When a model includes a co-performer, they are responsible for their own record keeping of consent and age verification. SextPanther is not responsible for obtaining such information, nor is SextPanther responsible for keeping such records.

Once you and your co-performer complete this form, you must keep a copy of it and their valid photo identification in the event SextPanther requests this information from you. As a best practice, you should also give your co-performer a copy for their records.

How to Complete This Form

1. Download a copy: Co-Performer Release Agreement form.

2. Open with a PDF viewer (e.g., Adobe, Preview).

3. Where you see the shaded text (e.g., YOUR FULL NAME HERE), select it, delete it, and type the appropriate information.

4. Where there is space for a photo ID, obtain a copy from your co-performer and attach to the agreement.

5. Ensure both you and your co-performer sign and date the agreement.

6. Keep the completed agreement, along with copies of the photo ID, in a safe and secure place.
